Preliminary Evaluation
Before starting treatment, the couple’s medical history, previous tests, and any past treatment processes are evaluated in detail. In women, ovarian reserve, hormone levels, and the structure of the uterus and ovaries are examined; in men, findings related to reproductive health, especially sperm analysis, are reviewed. After any additional tests deemed necessary are planned, all results are evaluated together, and the most suitable personalized treatment process is created for the couple.
Which Tests Are Requested Before IVF Treatment?
Before IVF treatment, the reproductive health of the couple is evaluated together. In women, hormone tests, AMH, ultrasound evaluation, and, when necessary, tests such as a uterine film are among the examinations; in men, sperm analysis is one of the basic evaluations. In addition, blood tests and infection screenings may also be planned for treatment safety.
Before Starting Treatment
At İRENBE IVF Unit, certain documents and procedures must be completed before treatment so that the treatment process can proceed safely and completely. In this context, a document showing marital status, photocopies of the patient’s and spouse’s identity documents, recent passport-style photographs, test results requested by the physician, and necessary consent forms should be prepared. It is important that all official and administrative processes are completed before egg retrieval and embryo transfer.
Starting Treatment
The IVF treatment protocol is planned individually, taking into account age, ovarian reserve, body mass index, lifestyle habits, and previous treatment history. Treatment usually begins in the first days of menstrual bleeding with ovary-stimulating medications. Medication use and the points to be considered during the process are explained in detail by our nurses.
Continuation of Treatment
After the treatment protocol is determined, a follow-up process lasting approximately 10–12 days begins. During this period, ultrasonography is performed at certain intervals, and hormone analyses are evaluated when necessary. According to the ovaries’ response to treatment, medication doses and the treatment plan are adjusted individually by the physician. The aim is to safely monitor the development of eggs in an appropriate number and quality.
Insufficient Response to Treatment
In some patients, despite an appropriate treatment protocol, the ovaries may not give the expected response. In this case, the process is re-evaluated by the physician; if necessary, medication doses, the treatment protocol, or the path to be followed are replanned individually.
Egg Triggering and Egg Retrieval
When the eggs reach the desired maturity, a trigger injection is administered, and the egg retrieval procedure is performed approximately 34–36 hours later. The procedure is carried out vaginally under ultrasound guidance and with anesthesia support. The collected follicular fluids are evaluated in the embryology laboratory, and the eggs are separated.
Laboratory Stage
The collected eggs are evaluated under special conditions in the embryology laboratory. Depending on the patient’s condition, conventional IVF or microinjection (ICSI) is applied to mature eggs. Fertilization status is usually checked the following day. Fertilized embryos are monitored in special culture media that mimic the uterine environment and in advanced incubator systems. Embryo development is evaluated on specific days, and the most appropriate transfer time is planned.
Embryo Transfer
Embryo transfer is the process of placing embryos, whose development has been monitored during the laboratory stage, into the uterus at the appropriate time. Depending on the patient’s condition, the transfer may be planned as a fresh embryo transfer or a frozen embryo transfer. In suitable patients, fresh embryo transfer is usually performed on the 3rd day after egg retrieval, or in some cases on the 5th day at the blastocyst stage. The transfer procedure is mostly painless, generally does not require anesthesia, and is performed under ultrasound guidance with the help of a special catheter.
